Your Department of Veterans Affairs has a form very every occasion. You'll be surprised to learn that few forms are required when a veteran applies for basic VA benefits such as pension and disability compensation.

VA Form 21-526

The veteran must complete the VA Form 21-526 one time and one time only. Once that's done, he or she may file additional claims, appeal denials and so on by writing a simple letter to the VA Regional Office that serves them.






VA Form 21-4140

Veterans who are rated as TDIU must complete this form each year. In theory, VA will mail the TDIU veteran the 21-4140 form on or about the anniversary date of the rating award. This is very often not the case however. The hapless veteran doesn't know to complete the form, the VARO forgets to mail it or it goes to a wrong address and the next thing the veteran knows, he or she receives a letter of proposal to reduce the rating benefit because the VARO didn't receive the form.
